Which means somewhere there's a Pappa Azagthoth and a Grandpa Azagthoth.
That thought fills me with terror and ecstacy.

A girl who rode my bus in high school's name was "Tanya," first syllable pronounced like the color "tan." WTF?
Where I'm from, if you pronounced that name any way other than that, you'd get funny looks.

I do have a friend who goes by "Tanya Cheex," so, you know, whole puns depend on it!

Names are really weird and I'm not very comfortable with them.
Names are the best. That is a fact.
Anyhow, here is a useful website that addresses many of your nickname/diminutive questions:

Does Oingo Boingo count?
I was going to say The The, so I vote yes.

Now I'm out of ideas. Every now and then I think of someone like Roger Rogerson or Jerome K. Jerome and then curse them for not being musicians.

Ha! My first thought exactly, quickly followed by "oh, not a musician" and then by "oh, no one will know who he is"

As big a Cure fan as I am, I don't really know a lot about then biographically and I'd never heard of Andy Anderson. The fact that one of the whitest bands in the history of music once had a black drummer blows my mind. I thought of the name earlier, but then I realised I was thinking of Angry Anderson.

He was there in all their biggest videos though - Caterpillar, Lovecats etc.

Roger Rogerson IS a musician.

Andy Anderson is another one of those weird Mitch Mitchell kind of situations though. His name is Clifford Leon Anderson. "Andy" is a diminutive of his last name.

Blue Murder
? It was awesome.
Underbelly
, less so. Too much glorification of crime.
Let me know if you have trouble finding it.

Jim is really James, so Jim James almost fits the bill.
Unfortunately, that dude is just pretending to have a first name incorporated into his last name, which is really Olliges. Truly, there is no subterfuge Wikipedia cannot uncover.

At least it doesn't say James James on his birth certificate. That would be a tragedy.
